The Zentorno is heavily based on the Lamborghini Sesto Elemento, evident in its unique silhouette, side profile, distinctive mirrors, hexagonal rear vents accompanied by dual roof vents, and the characteristic front fascia. Additionally, its design is significantly influenced by the Lamborghini Veneno, as reflected in its grille, sharper front end, hood vents, rear vents, roof vent design, diffuser, and the painted accents on the wheels and front diffuser. The "Racing Spoiler" tuning option draws inspiration from the Veneno's spoiler, albeit featuring double mounts and fins. The C-pillar vents appear to be derived from the Lamborghini Egoista. The hexagonal exhausts are inspired by the Lamborghini Aventador J, while the shape of the rear bumper is taken from the Aventador LP 700-4. The "Hyper Exit Exhaust" option is also based on the LP 700-4. The tail lights seem to combine features from the Veneno, Aventador, and Lamborghini Huracán, while the rear design incorporates aspects from the Sesto Elemento, Veneno, Aventador, and Lamborghini Gallardo LP 570-4 Super Trofeo Asia. The rear might have also been influenced by the DMC LP988-4 "Edizione GT" body kit for the Aventador, which is based on the aesthetics of the Sesto Elemento and Veneno. The headlights are modelled after the 2011 Acura/Honda NSX Concept.
Certain default colour schemes of the car also echo the Sesto Elemento, as they resemble its signature carbon-fibre-and-red coating, although substituting the colour red with orange, potentially alluding to the Lamborghini Gallardo LP 570-4 Edizione Tecnica.
*The "Kisama" livery added in The Criminal Enterprises update is based on a Lamborghini Huracan wrap created by Khyzyl Saleem. **Furthermore, two similar liveries named "Yellow Stripes" and "Street Kisama" are available for the Deveste Eight and Torero XO, respectively. *The "Comic Book Racing" livery added in The Criminal Enterprises update is based on the GEAR Racing Lamborghini Huracan GT3 livery by Andy Blackmore Design.
